Basic Eye Examination includes Visual Fields and Glaucoma Screening

The following is the current rate for those patients who are not insured under the Ontario Health Insurance Plan (OHIP).

New patients	$75.00
Previous patients	$75.00

The basic exam is required prior to a dilated fundus exam.

The basic exam is the required first part of a Diabetic exam and is covered by OHIP. The dilated fundus exam is the second part of the Diabetic eye examination.

Follow up eye exams:	$30.00

Dilated Fundus Exam including:	$40.00

Medical doctor's report

All patients receiving a dilated fundus exam will be required to remain in the office for one hour and have arranged transportation.

Prescription Update:	$30.00
New patients will be requested to sign a release form.
Forms:	$30.00
Ministry of Transportation, Ontario Provincial Police, etc...
Medical Doctor's Report:	$30.00
Legal / School / Third Party Report:	$40.00
All patients are requested to supply a list of medication(s) and allergies.
Payment is required prior to exam. (Cash or Debit)
All listed fees are for non-insured patients.
All patients paying for their eye exams will be issued a receipt that can be submitted to their Insurance Company or retained for income tax purposes .
All exams at this office are performed without contact lenses.
